Clare Clapham

Clinical Operations Manager

Clare joined the industry at the tender age of 16 having worked as a volunteer whilst completing her BTEC, progressing on to work as a Support Worker with people with Learning Disabilities. She then trained as a General Nurse in 2003 and worked in Haemotology, Oncology, and as a District Nurse before then joining the CHC Commissioning Team.

As one of the case managers for ICB that worked closely with Unity Plus, Clare case managed the service users with some of the most complex health needs in the area. Clare worked closely with Unity Plus as a commissioner and loved the passion the team exhibited so much, and saw the improvements made by the people we support that she was motivated enough to agree to join the team in November 2022.

How long have you been at Unity Plus?

I joined Unity Plus in November 2022.

Tell us about your career history?

My extensive career in health and social care began with volunteering and supporting people with learning disabilities. I then trained as a General Nurse and worked in various settings, including York Hospital in Leeds, as a Community Nurse and later as a Chemotherapy Nurse.

My experience expanded into case management with the Continuing Healthcare team, where I managed complex health needs. This combination of clinical experience and case management skills led me to my current role as Clinical Operations Manager at Unity Plus.

What qualifications do you hold?

– BTEC (Health and Social Care)
– DipHE Social and Professional Studies: Learning – Disabilities
– Registered General Nurse (RGN) qualification
– British Sign Language Level 1
– Extensive experience in case management and continuing healthcare.

Claire’s Responsibilities

Oversee Unity Plus’ clinical operations, ensuring the highest standards of care. I manage the team, support the development of individual care plans and risk assessments, and ensure compliance with regulatory requirements.

Provide guidance to the team in addressing complex health needs, particularly for individuals with learning disabilities and other significant conditions.

Collaborate with other healthcare professionals to ensure holistic, person-centred care for those we support.

How does your role support Unity Plus’ mission?

My role is integral in supporting Unity Plus’ mission by ensuring that people receive compassionate, high-quality, and personalised care.

By overseeing the clinical and social aspects of care delivery, I help ensure that every person supported by Unity Plus receives the best possible care tailored to their needs.

This aligns with our goal of promoting independence, well-being, and social inclusion for all individuals.

What are your hobbies?

In my spare time, I enjoy wild swimming, hill walking, and cycling. These activities help me stay fit and connect with nature, which I find energising and refreshing.

I’m a passionate motorsport enthusiast and regularly travel across the UK to attend British GT and BTCC races. It’s taken me to some of the country’s most iconic circuits and is a great way to combine travel with something I love.
