We aim to provide the right care at the right time, tailored to enable individuals to thrive in their community, at home, or over the phone. Our supported living service covers Leeds, York, Harrogate, Knaresborough, Skipton, Scarborough, Wakefield, Ripon, and Greater Manchester, providing support for vulnerable adults with the following complex care needs:

Selecting the right supported living provider is a significant step. At Unity Plus, our team boasts a diverse range of specialist skills and backgrounds in mental health, commissioning, neuro rehab, learning disabilities, and nursing. Our service is regulated by the Care Quality Commission (CQC) and subject to extensive self-assessments and audits, ensuring that the service we deliver meets the highest standards. We ensure all staff have undertaken 30 modules of training before they start with us, which is much higher than other vendors.

We have recently been awarded 100% in our recent Quality Review by North Yorkshire Council, scoring the maximum 190 points across eight different domains.

What we offer as supported living providers


Our team listens to each individual’s story, routines, needs, and preferences. This forms the foundation of an assisted living support plan, which will be created before the start date of support.

HEALTH & MEDICATION: Specific health conditions, medication schedules, allergies, dietary requirements, and how health needs are to be monitored and supported.

FINANCES: How the individual manages their finances (if support is required), who is responsible for what, and specific instructions regarding financial matters related to their care.

DAILY ROUTINES: When things usually happen, where they happen, and the preferred ways of doing things.

PREFERENCES: Which things to buy, which things to avoid (food, activities, sensory sensitivities etc).

HOME MAINTENANCE: Tasks the individual needs support with, preferred cleaning products, and any specific requirements for their living environment.

COMMUNICATION: What words to use, language preferences, any communication aids, and how to understand the individual.

We specifically match each individual with a Support Worker, support team, and a dedicated on-site Key Worker, supported by an allocated Team Leader chosen due to their specific skills and interests.

We take the time to find connections between an individual’s likes, dislikes, and interests. Having the right team that aligns with an individual’s preferences provides a positive and trusting relationship.
